Important Instructions - please read:
Thank you for your interest in adopting one of our rescued Siberian Huskies. Please read our Adoption Requirements, then fill out this form as completely as possible. Please be thorough - incomplete applications will be rejected. Most fields are required. If a particular question does not apply to you, enter "N/A". Some fields already have "N/A" entered - replace that with your answer when applicable. If you are a minor, please have your parents fill out the applications, as we cannot accept applications from minors. Please note that we do not place dogs in homes outside Arizona, due to the difficulty in screening out-of-state applicants.

Once you have answered all the questions, read the statement about Sibes never being allowed off-lead in an unsecured area and check "I Understand". Finally, enter "YES" (in upper-case) at the bottom to agree to our adoption terms and conditions, then click Submit. One of our volunteers will then review your app and get back to you as soon as possible. You will also receive a confirmation email to let you know your app was received. However, this is not an automatic process and it may take up to two days to receive this email. Be sure your email spam filters accept email from adoption@ashra.org, so our emails don't get marked as spam.

Note: If you are not familiar with the characteristics and traits of the Siberian Husky, we encourage you to do so before completing this application. Doing so will save both your time and ours as you may decide that another breed of dog is more suitable.

Arizona Siberian Husky Rescue and Adoption requires, in all cases, that any unaltered dog adopted through it’s auspices be neutered or spayed before adoption unless there is a physical reason for delaying such surgery. We also require that the adopter keep all shots and vaccinations current. All other dogs in an adoptive home must be spayed/neutered. I WILL NOT ATTEMPT TO HOLD ARIZONA SIBERIAN HUSKY RESCUE AND ADOPTION RESPONSIBLE FOR ANY DAMAGES WHICH THE ANIMAL MAY DO TO ANY PERSON OR PROPERTY, NOR FOR ANY ILLNESS OF THE DOG OR DEBTS OR PAYMENT INCURRED FOR SERVICES OF A PRIVATE VETERINARIAN.

ASHRA requires a non-refundable application fee of $10. Your application will NOT be processed until we receive notification that your application fee has been received.

There is an adoption fee for each dog adopted, typically of $150, which must be paid before the adoption is finalized. The application fee can be applied towards the adoption fee. I UNDERSTAND THAT MY APPLICATION FEE IS NON-REFUNDABLE AND CAN BE APPLIED TO THE ADOPTION FEE IF I AM APPROVED FOR ADOPTION. I UNDERSTAND THAT AN ADOPTION FEE IS REQUIRED FOR EACH DOG ADOPTED.
